    Did the final 10k stage on Caraboks. Turned it in a received the evade reward but the achievement did not unlock.

    I needed to go twice to terminal, no need to find carabok mission again, just click on terminal and will unlock

    This fixed the problem.
    Had to use the terminal 2 - 3 times and it unlocked.

    You might expect same happen with gallard, I recall having same issue.
